Oblique metaphyseal osteotomy with rigid internal fixation of the fifth metatarsal of the left foot. 2. Proximal interphalangeal joint arthroplasty with derotational oseteoplasty of the fifth digit of the left foot. The surgeon was Jaime A. Carboneell, D.P.M. and first assistant was Francisco Lopez Burmudez, D.P.M. Anesthesia: Monitored anesthesia care with intravenoues sedation combined with a local nerve conduction block consisting of a 50:50 mixture of 2% lidocaine plain with 0.5% Marcaine plain totaling 10 cubic centimeters were infiltrated into the patient's left foot in the form of a fifth ray block. Hemostasis: For hemostasis, a left above ankle pneumatic tourniquet was inflated to a pressure of 250 millimeters of mercury after an Esmarch bandage exsanguination of the left lower extremity was performed.
